Hollows, shadows, puffiness and skin quality require different answers

The under-eye area can look tired for many reasons, including natural anatomy, volume change, visible vessels, pigmentation, fluid retention and thin skin. Because several of these can appear together, the treatment that helps one person may make another person look heavier or more swollen.

  • A hollow or shadow beneath the inner eye
  • A visible groove between the lower eyelid and cheek
  • Fine, crepey skin
  • Darkness that remains despite adequate sleep
  • Puffiness or under-eye bags

Why the under-eye changes

The lower eyelid and midface form one connected anatomical area. With time, changes in the orbital rim, fat compartments, cheek support and skin quality can make the lid–cheek transition more visible. Pigment, visible blood vessels and shadow can all look like a “dark circle,” yet they require different approaches.

Why careful assessment matters

Adding filler to a true hollow may soften a shadow, but filler cannot remove pigment or improve every type of under-eye concern. Where puffiness is already present, additional volume may create a heavier appearance.

How LIVO assesses the eye area

We look at the under-eye in relation to the cheek, temples and brow. We assess hollowing, skin thickness, pigmentation, visible vessels, puffiness, previous filler and how lighting changes the appearance.

  • Dermal filler — may be considered for selected hollows or midface support after careful anatomical and risk assessment.
  • PDRN skin rejuvenation — may support skin quality around the eye in a product- and protocol-specific plan; it does not replace volume or remove eye bags.
  • Wrinkle relaxers — may soften crow’s feet created by muscle movement; placement around the eye requires precise assessment.
  • Skin-quality care — appropriate skincare, microneedling or other selected procedures may address fine texture, depending on the area and device.

Daily skincare and sun protection

Morning — use a gentle hydrating product, then protect the area with broad-spectrum SPF 30 or higher and sunglasses. A sunscreen that does not sting the eyes is more useful than one you avoid applying.

Evening —. Choose products specifically formulated for the delicate eye area, with a focus on hydration and maintaining skin moisture. Introduce a low-strength retinol slowly and stop if irritation persists. Keep stronger facial acids and retinoids away from the eyelid margin.

Ingredients worth looking for — glycerin, hyaluronic acid and ceramides for hydration; niacinamide for barrier support and uneven tone; vitamin C in a well-tolerated formula; caffeine for a temporary improvement in the look of puffiness; low-strength retinol for fine lines.

Eye-area skincare may improve dryness, fine texture and some surface discoloration. It cannot remove protruding fat pads or replace meaningful structural volume.

Patch-test new products and introduce one active ingredient at a time. Apply sunscreen generously about 15 minutes before sun exposure; when outdoors, reapply at least every two hours and sooner after swimming, sweating or towel drying.

Will under-eye filler remove dark circles?

Only when shadow from a suitable hollow is an important contributor. Pigment and visible vessels will remain.

Can eye bags be dissolved with filler treatment?

No. Filler adds support or volume; it does not remove protruding fat pads.

Why might under-eye hollowness involve discussing cheek support?

Mid-face support can affect the transition between the lower eyelid and the cheek. If the hollowness is related to a lack of cheek support, directly filling the under-eye area may not always be the safest or most natural-looking approach.
